The European Committee for Antimicrobial Susceptibility Testing (EUCAST) recently proposed epidemiological cut-offs (ECOFFs) for a limited number of antimicrobial compounds and for Campylobacter jejuni and Campylobacter coli only. The variability in the results obtained with these methods hampers setting epidemiological cut-off (ECOFF) values and clinical breakpoints according to EUCAST guidelines. Methods for susceptibility testing and determination of the minimal inhibitory concentrations (MICs) need to be standardized for MTBC isolates for old and new agents. However, the ECOFF values determined using one or the other of the methods coincide to within one dilution step.

• The ECOFF – The ECOFF distinguishes between organisms without and with phenotypically expressed resistance mechanisms for a species and a drug in a defined test system. – Within a species, it is the highest MIC of organisms lacking phenotypically expressed resistance. – Organisms without resistance mechanisms are not by default EUCAST ECOFF: 23 mm . Median ± 1 mm from reference distribution .

For E. coli, the IMM ECOFFs were mostly similar to the EUCAST ECOFFs, with only minor differences (Δ = 1 mm), e.g., 19 mm versus 18 mm for cefuroxime. EUCAST also show cutoff values, ECOFF values, which were used as the reference. The NRI generated +2.0 SD values showed the best agreement with 26 of 27 within ±1 dilution step and 17 exactly on the ECOFF values for Staphylococcus aureus , and 25 of 27 within ±1 dilution step and 14 right on the ECOFF values for Escherichia coli .

This study demonstrates that ECOFF-based analysis of AST data of only four aminoglycosides provides accurate information on the resistance mechanisms in E. coli. Since aminoglycoside resistance mechanisms, despite having in certain cases a minimal effect on the minimal inhibitory concentration, may compromise the bactericidal activity of aminoglycosides, prompt detection of resistance
